Provider and Course Listings
The following Continuing Education reports are available. Please select a report, complete the requested information, and submit your request.
- Approved Providers Listing — displays the addresses and phone numbers of approved providers.
- Approved Courses by Providers — displays names, credits, methods, and subjects of approved courses by providers.
Approved Providers Listing
You may enter either a Provider ID or Provider Name.
- To look up a specific name, you may enter the initial letters of that name in the “Provider Name” field.
- To obtain a list of providers whose name contains a specific word, precede it with a “%” (for example, enter “%Life” in the “Provider Name” field to obtain a list of all providers that have the word “Life” in their name).
- For a complete list of all providers, leave both fields blank.
